A DMCA counter-notification must include the following:
- Your physical or electronic signature.

- A statement under penalty of perjury that you have a good faith belief the material was removed or disabled due to a mistake or misidentification.
- Your name, address, phone number, and a statement consenting to the jurisdiction in which you are located.
- A statement agreeing to accept service of process from the person who filed the original infringement notice or their agent.
